The DS-11 Passport Form must used form if:

• You have never had a U.S. passport or
• You are under age 16 or
• Your most recent passport was issued more than 15 years ago or
• Your most recent passport was lost or stolen. (If you are applying because your most recent, valid passport was lost or stolen, you will also need to submit Form DS-64 listed on the left.)

Please follow instructions on pages 1 and 2 and read carefully pages 3 and 4.  Complete both pages of the application and print on one-sided pages (do not sign the application).  Bring pages 3 and 4 of the instructions along with the application when you appear at the acceptance facility or passport agency to apply for your passport. (Do not sign the DS-11 application form until the Passport Acceptance Agent instructs you to do so.)
